Vital Records
• A certified copy of a Birth, Death or Marriage license can be obtained in person during business hours or by mail.
• Check or money order should be made payable to the Town of North Andover.
• Please include a STAMPED SELF-ADDRESSED ENVELOPE
Birth Certificates
Certified copies of birth certificates can be obtained from the Town Clerk’s office. Birth certificates are on file for children whose parent(s) were residing in North Andover at birth.
The fee for a birth certificate is $8.00 per certified copy. Please include the full birth name, date of birth and name of the mother for any requests.

Death Certificates
Certified copies of death certificates can be obtained from the Town Clerk’s Office. Death certificates are on file for decedents who were pronounced in the Town of North Andover, or who resided in the Town of North Andover at the time of death. Persons who died out of state will not have a record on file in the Town of North Andover.
The fee for a death certificate is $8.00 per certified copy. Please include with your request the decedent’s full name and date of death.

Marriage Certificates
Certified copies of marriage certificates can be obtained at the Town Clerk’s office. The record of marriage will only be here if the bride and groom filed for the marriage intention (application) in the Town of North Andover.
The fee for a marriage certificate is $8.00 per certified copy. Please include with your request the bride’s maiden name and the groom’s name as well as the date of marriage.
