Guidelines for applying for a US Passport
The following is required when applying for a US Passport
1. Proof of US Citizenship:
• Long form birth certificate with the raised seal
• Certificate of naturalization
• Certificate of citizenship
• Report of birth abroad
2. Proof of Identity:
• You must establish your identity to the satisfaction of the acceptance office example: drivers license
3. Two recent color passport photographs 2x2 in size. Authorized passport photo agencies in this area are CVS, Kenyon Photo or AAA.
4. Fees CHECKS ONLY
* Over 16 years of age: $67.00 to the US Department of State
* Under 16 years of age $52.00 to the US Department of State
* PLUS an Execution fee of $30.00 for each person to the Town of North Andover. This is for both children and adults applying for a US Passport
IF YOU NEED AN EXPEDITED PASSPORT THE EXPEDITED SERVICE FEE IS AN ADDITIONAL $60.00 PER PERSON
BY LAW, THE PASSPORT PROCESSING, EXECUTION, AND SECURITY FEES ARE NON REFUNDABLE.
Each minor child under the age of 14 applying for a passport must appear in person with both parents and or guardians. Long form birth certificates with the raised seal are required. If only one parent is available then a notarized written statement consenting to the passport must be included with the application. Sole parents must submit proof of sole authority.

US Passport renewal information
If you answer yes to the following questions you can use the DS-82 renewal form and process your own application.
1. I can submit my most recent US passport.
2. I was at least 16 years old when my most recent US passport was issued.
3. I was issued my most recent US passport less than 15 years ago.
4. I use the same name as on my most recent passport.
OR
I have had my name changed by marriage or court order and can submit proper identification to reflect my name change.
Passport renewal applications are available on-line at travel.state.gov or forms are located at Town Hall for your convenience. As an authorized passport agent we only process new applications. In addition, contact the National Passport Information Center at 1-877-487-2778 or by email at NPIC@state.gov.
